Hope the New Year is finding you well!

Here are some publications from the National Diabetes Education Program. This website offers many publications on a wide variety of topics. You can order reprints for a price or simply print them off yourself for free. These publications help clients think about how to start making changes in their lifestyle (eating, exercise, care, etc) to increase their chances for success.

The first is tool, "Make a Plan" helps clients start to plan by asking questions about why they would want to make these changes, what has stopped them in the past, and what they want to achieve. It would be a useful tool for you as well when trying to do some motivational inteviewing with clients.

The second is "Small Steps. Big Rewards. Your GAME PLAN to Prevent Type 2 Diabetes: Information for Patients" It helps with how to set goals and the rewards reaped by small changes.

The third is "Power to Prevent: A Family Lifestyle Approach to Diabetes Prevention" This is a good publication to use in a group setting and to help patients with family on how to incorporate lifestyle changes. It was written and targeted for African-American families as their risk for diabetes is almost twice that of white families, but the information, such as tips to get whole family involved, is useful to all. It also has many helpful tools such as "Four Keys to Healthy Eating Out".
